The County Planning System is no longer accepting Schedule C applications from Certified and Operational State Aid Funded programs applying for Capital Grant Financing.  To apply for Capital Grant Financing, not-for profit voluntary providers must use the Schedule C form posted on the OASAS website (here).  The completed Schedule C form is then to be submitted by the provider to their respective OASAS Regional Office for consideration.
