Attached please find revised list of vaccination sites for week FOUR of the State’s vaccination rollout.
**Please note: The links in the attached document are unique and should only be used to schedule vaccination appointments for priority populations. Please do not forward or redistribute. This message was sent to all OMH provider agencies operating outside of New York City. Receipt of this message does not necessarily mean that any particular program operated by the recipient agency meets the criteria in the guidance memo.
You will also find attached revised guidance, effective January 4, 2021, from the NYS Department of Health for the Prioritization of Essential Health care and Direct Support Personnel as well as High-Risk Populations for COVID-19 Vaccination.
The attached guidance expands the list of individuals eligible to make appointments to receive the vaccine. Of note, all Outpatient/Ambulatory front line, high risk health care providers who provide direct in-person patient care or other staff in a position where they have direct contact with OMH service recipients, regardless of age or medical status, are eligible to make an appointment. At this time only staff in these settings are being prioritized, not the patients or service recipients being treated/served.
The list of programs included in the outpatient/ambulatory category is found below, but it must be clearly understood that staff in these programs that are in telecommuting status, or are delivering services virtually, from their homes or work locations, are not eligible to make an appointment at this time.
The list of programs eligible includes:
- Clinic/CCBHC
- PROS
- ACT
- Mobile Crisis
- Partial Hospitalization
- Day Treatment/Continuing Day Treatment
- Drop-in Center/Recovery Centers
- Peer Support Programs
- Bridger Case Management/Care Coordination
- HCBS support services
Please review this guidance document in its entirety and identify priority populations within your workforce and the population which you serve in accordance with this guidance.
