Please review the attached guidance regarding the changes to the HCBS Level of Care Eligibility Determination for the Licensed Practitioner of the Healing Arts (LPHA) form process. The attached guidance outlines what should occur in the Unified Assessment System (UAS) since this change has not yet been made.
