Making Integration Possible with a System of Care Approach

The COSOC (Co-Occurring System of Care) framework developed in the Mid-Hudson Region has brought together a wide range of community partners to create integrated care opportunities for individuals with mental health, substance misuse, addiction & other complex challenges, and their supports.

Presenters:
Stephanie Marquesano, Founder and President, The Harris Project
Michael Orth, Commissioner, Westchester County Department of Health
Dr. William Mullane,  Director of Innovation, Integration, and Community, Partnerships, Westchester Mental Health Jewish Community Services

Objectives:
• Participants will learn about goals and objectives of the Mid-Hudson COSOC project through a lived experience, Local Government Unit, and provider lens.
• Participants will be able to identify steps and considerations for a system of care approach from an organization and larger system perspective.
• Participants will leave with a grasp of potential deliverables and outcome measures of providing integrated co-occurring care.
• Participants will explore additional opportunities and barriers.

The Partnership’s Learning Opportunities are designed to highlight innovations, in-care achievements, and provide a learning space for providers and community members to share how to improve pathways to integrated care and support connections with those shifting the approach to care for their communities.
