The survey tool related to the OASAS Restructure Addiction Services Financing Project (RASFP) will be sent to all OASAS-funded providers on Tuesday, December 12, 2023.   The email will come directly from the consultant, Third Horizon Strategies, this week, and will provide a link to the survey application.  We are requesting that we receive only one survey response from each provider.  Also, it will be important for your fiscal person to complete the survey within two weeks of receiving the link – or no later than December 29, 2023. 

The survey will ask questions about each of your program types and the State aid funding those programs.  The survey should take 10-15 minutes to fill out.  However, for providers that receive OASAS State aid support for numerous program types, the survey will take longer.  The survey does not time out.  If you need to leave the survey, the application will bring you back to where you left off responding to the survey when you click on the link again.  If you experience any technical difficulties with the survey application or have questions, please send them to OASAS at mailbox: RASFP@oasas.ny.gov.
