Subject: OASAS Stakeholder Survey

Dear Providers, 

We are conducting a survey of OASAS providers to help identify and address gaps in the provision of services within the New York State continuum of care for substance use related services. Our goal is to collect your thoughts and experiences regarding barriers to services along this continuum.  Your feedback is very important for the next steps in this project, selecting and implementing specific best practices to improve outcomes of all our consumers. This survey is completely anonymous, and we will not ask you for any individual identifying information.
