 The Partnership for Integrated Care is conducting a brief survey to better understand levels of integrated care in agencies across the state.  We are asking each agency to complete one survey to help inform our future training , project activities, advocacy agenda and office hours.  The survey is to better understand your agencies level of integrated care, or the extent to which your organization addresses individuals who have physical health, mental health and substance use treatment needs.
