Last week I received a copy of the SCN PowerPoint presentation DoH officials have updated (as of November 2024) and are now using for the latest round of stakeholder meetings. I sent it to all members. It is attached to this email for your convenience. Ideally there would be a red lined version that identifies any changes made from the previous deck to this one, but I’m still searching for it.

RESOURCES:
New York State is working to implement the New York Health Equity Reform (NYHER) 1115 Waiver Amendment, including initiatives to advance health equity, improve population health, and support the delivery of health-related social needs (HRSN) services.
Providers can connect to this work through regional Social Care Networks (SCNs):
Community-based organizations and behavioral healthcare providers are encouraged to connect and collaborate with regional SCNs to help screen, navigate, and deliver health-related social needs services to Medicaid members. OMH encourages providers to ensure they are included on SCN directories, which will bring awareness to available services and help navigators facilitate referrals.

WEBINAR:
The New York State Department of Health (NYS DOH) is hosting a webinar on December 12, 2024, from 2:00-3:00pm ET for providers on the State’s new Social Care Network program. SCNs are regional networks that will bring together community-based organizations and providers to help eligible Medicaid Members access health-related social needs services, such as housing supports, food and nutrition assistance, transportation, and more.

What information will be covered? This webinar will overview the SCN program and share opportunities for providers on the role they can play – from referral, to screening, clinical criteria attestation, and service delivery.
Who should attend? This webinar is intended for providers and leaders involved in population health management, social care, and/or care management from the following types of healthcare organizations:
· Hospitals/Health systems
Primary care practices
Behavioral health organizations treating mental health and substance use disorders
Community health centers, including federally qualified health centers (FQHCs)
While the webinar will not cover detailed operational guidance, this information will be available from SCNs as health care providers join networks. This webinar is not intended for Medicaid Members or advocates – separate sessions and information will be shared closer to program launch.
