BH IPA Partners:

New York State is excited to announce the launch of a value-based funding opportunity, the BH IPA Hospital Discharge Planning Initiativepending CMS approval. This initiative is intended to improve statewide hospital discharge planning and connection to community services in alignment with other NYS initiatives.

Please note: 

  • All BH IPAs are eligible to apply for this initiative, either individually or as a group.
  • To be considered for an award, BH IPAs, either individually or as a group, must meet or exceed a minimum Medicaid managed care recipient member attribution threshold. Applications below this threshold will not be funded. More details are available in the Program Guidance (attached) and will be discussed during the technical assistance walkthrough, registration linked below.
  • Eligibility and awards will not be made prior to the beginning of Performance Period 1, which starts April 1, 2024.

Subject: BH IPA Funding Opportunity: Update

BH IPA Partners:

This is a status update on the potential BH IPA funding opportunity: Hospital Discharge Planning Initiative (announcement attached). This initiative is intended to improve statewide hospital discharge planning and connection to community services in alignment with other NYS initiatives.

Although CMS approval has not yet been received, the application process for this potential funding opportunity will be available in the next few weeks. Eligibility and awards will not be made prior to the beginning of Performance Period 1, which starts April 1, 2024.  

Payment Arrangement Details:

• Payment 1 – comprised of 70% of total award released upon contract between awardee and MCO. To retain this payment, the awardee must improve from baseline for at least one performance measurement goal in at least one system (either mental health or addiction) during Performance Period 1 (PP1) April 1, 2024 – September 30, 2024.

• Payment 2 – comprised of 30% of total award. BH IPAs must retain progress within a NYS-defined range on at least one PP1 goal AND improve or retain progress from PP1 on at least one goal from the other domain (mental health or addiction disorder) to retain remaining 30% of award, during Performance Period 2 (PP2) October 1, 2024 – March 31, 2025.

Performance measures:

There are two substance use disorder and two mental health measures:

  1. Initiation of Pharmacotherapy Upon New Episode of Opioid Dependence (POD-N)
  2. Follow-up After High-Intensity Care for Substance Use Disorder (FUI) (7-Day Follow-up)
  3. 7 Day Follow-up after a Mental Health Hospitalization
  4. Engagement in Community Based Mental Health Care After a Mental Health Hospitalization

The State will release the Technical Specifications detailing the definition, numerator, and denominator for these four performance measures soon.

Upon release of application, NYS will provide technical assistance regarding funding structure, application, and program guidance.
