The following note went out yesterday to everyone on our Partnership for Integrated Care listserv from Dr. Virna Little who is coordinating this initiative on behalf of the Partnership (NYS Council and InUnity Alliance).  Please see below and attached for information on our upcoming learning community ! 

The Partnership for Integrated Care is continuing efforts to further integration in our system. Beginning in May, we are offering a series of learning collaboratives to help agencies move the needle in terms of attaining greater integration of care. The first one will focus on those agencies that are just starting their journeys into integrated care. Subsequent learning collaboratives will be offered that focus on agencies that are further along in their process to integrate.

WHO SHOULD APPLY ? Is your organization just starting the integrated care journey ? Are you thinking about what elements of integration you need to prioritize and how to prepare your team ? This learning community will give you the information and foundational steps you need to take your organization and team forward on a path to integrated care. Any organization who is currently providing mental health, substance use or primary care services and is interested in adding services to become a more integrated care provider is a candidate to apply.
